A lot of my patients that have diabetes and neuropathy tend to develop ulcers on the front of the big toe as you can see in this picture. This is normally caused by the biomechanics of your foot. Since the big toe cannot move very well you develop a callus in the front of the toe. The best way of treating this after trying different types of shoes and having them become ineffective is to actually increase the movement of the joint over the big toe joint. This can either be done by cutting the tendon that pulls down the big toe joint or by removing a portion of the bone to help the big toe joint move better.
